Prior Authorization Letters

When Cleo generates a prior authorization letter from the extension, you can open it in the Hub to review evidence, edit the letter, and export it for submission.

PA letters are generated by Cleo when you ask for help with prior authorization (e.g., “Generate a PA letter for Wegovy”). After generation, an attachment card appears in the chat. Click “Open in Hub” to access the full editor.

The Hub opens directly to the PA Letter Editor with all evidence and the draft letter loaded.

The Evidence Review tab shows all clinical evidence found to support the prior authorization:

Each piece of evidence includes:

  • Summary — What the evidence shows
  • Citation — Source document and date
  • Toggle — Include or exclude from the letter

Toggle evidence items on or off to control what appears in the final letter. Excluded evidence is dimmed but remains available if you change your mind.

The Edit Letter tab provides a text editor for the generated letter:

  • Summary Section — The clinical justification for the medication
  • Evidence Section — Auto-generated from included evidence items
  • Conclusion — Medical necessity statement

You can edit any section directly. Changes are saved automatically.

The Export tab provides options to get your letter out:

  1. In your EHR, navigate to the patient’s chart

  2. Ask Cleo to generate a PA letter (e.g., “Write a prior auth appeal for Ozempic”)

  3. Review the results in the chat — Cleo shows evidence found and generates a draft

  4. Click “Open in Hub” on the attachment card

  5. Review evidence — Toggle items to include/exclude, verify citations

  6. Edit the letter — Refine the summary or conclusion as needed

  7. Export — Copy to clipboard or download as PDF

  8. Submit — Paste into your PA submission system or fax

  • Verify citations before submitting — ensure evidence actually appears in the referenced documents
  • Check dates — more recent evidence is generally more compelling
  • Include failure reasons — specific reasons why prior therapies failed (intolerance, ineffective, contraindicated) strengthen the appeal
  • Review quantity limits — ensure requested quantity aligns with typical coverage limits
