Patient Details

Click any patient card in the Hub to open their detail view. This page consolidates clinical data from all connected EHR systems into a single view.

At the top, you’ll see the patient’s name, age, gender, and MRN alongside a risk tier badge (Critical, High, Medium, or Low). The overview card shows key metrics at a glance: total alerts, care gaps identified, when data was last updated, and which EHR systems contributed data.

Source badges (like “Epic” or “Cerner”) indicate where each piece of data originated. If a patient receives care at multiple facilities, you’ll see multiple source badges.

The detail view organizes patient information into expandable sections. Click any section header to expand or collapse it.

Allergies appear prominently with red alert styling. Each allergy shows the allergen name and reaction severity. These are always visible regardless of scroll position.

Critical Alerts display high-severity clinical findings that need attention—things like dangerous drug interactions, significantly abnormal lab values, or urgent care gaps. These appear with red styling to stand out.

Active Conditions shows current diagnoses as visual chips. Each condition displays its status (active, resolved, or inactive). The section header shows the total count.

Medications lists all current medications with dosage and frequency. Medications with potential conflicts are flagged with a warning badge. You’ll see the drug name, dose (e.g., “10mg”), frequency (e.g., “once daily”), and route if available.

Lab Results appear in a table format with the test name, result value, reference range, and collection date. Abnormal results are highlighted—values outside the reference range show colored status indicators. The table header stays visible as you scroll through results.

Care gaps are recommended actions or screenings that may be overdue. These are identified automatically based on the patient’s conditions, age, and clinical guidelines. Common examples include:

  • HbA1c testing for diabetic patients
  • Blood pressure monitoring for hypertension
  • Age-appropriate cancer screenings
  • Eye and foot exams for diabetics
  • Vaccination updates

Each care gap shows what’s recommended and why it’s relevant for this patient.

From the patient detail view, you can:

Refresh — Click the refresh icon to pull the latest data from your EHR. A loading indicator shows while the refresh is in progress.

Invite to Halo — If the patient isn’t connected yet, send them an invitation to join Halo and access their health records.

Message — If the patient has accepted their Halo invitation, click to open secure messaging.

Remove — Remove the patient from your Hub. This doesn’t delete any EHR data—the patient will reappear if you view their chart again in your EHR.

When a patient has data from multiple EHR systems, the Hub consolidates it into a single view. Each piece of data shows which source it came from, so you can see the complete picture across facilities.

Duplicate information (like the same medication appearing in two systems) is merged intelligently. If there are conflicts between sources, both values are shown with their respective source labels.
